We are excited for Camp Chaos South Creek VBS 2025! VBS is June 16th-19th 6-8pm for kids ages 4 years old - entering into 5th grade. Each day we will have snack, craft and some play time! We are looking forward to seeing everyone there and teaching the kids about Jesus in fun and exciting ways! In the event of an emergency where I cannot be contacted or present, I consent to any reasonable medical treatment as deemed necessary by a licensed physician, surgeon or dentist. In the event that my child requires treatment by a physician and/or hospital personnel, I agree to hold such person free and harmless of any claims, demands, suits or damages arising from giving of such consent. I also acknowledge that I will ultimately be responsible for the cost of any medical care not reimbursed by the health care provider, I affirm that the health insurance information provided is accurate on this date and will, to the best of my knowledge, still be in force for my child named above until the end of the school year.
